Union County Community Health Assessment
Union County Consolidated Human Services Agency needs your help to determine the key factors that impact the overall health and safety of Union County residents. We have partnered with Novant Health, Atrium Health and other community partners to conduct a Community Health Assessment (CHA).
Your responses are anonymous and will be compiled with all responses received to help determine the physical, mental and environmental health needs of Union County residents.
